MEDICAL REVIEW
This category is for operational CORFs preparing to submit their initial billing, submitting on-going billing, or currently in Medical Review.
Medicare Part A Intermediaries will initiate a pre-payment, educational Medical Review to determine if Medicare guidelines are being met. Frequently the Medical Review is initiated with the first CORF billing, or shortly thereafter.
The CORF must submit information as required by Additional Documentation Requests (ADRs) from the Fiscal Intermediary (FI). Medical Records must support the billing submissions.
The length of Medical Review will be based on the CORF's billing utilization and denial experience, and may last from 2 months to one year! The percentage of claims in review can range from 25% to 100%!
While in Medical Review, payment of claims which are in Medical Review status will be withheld until the claim(s) are approved and released for payment. Claim(s) may also be partially or completely denied, resulting in either a reduced payment or no payment at all for those services your CORF has already provided!
The Medical Review Department at your Fiscal Intermediary will attempt to audit your claims as quickly as they can, depending on available staff and their other prioritized responibilities. Claims in Medical Review may not be released for payment for weeks, or even months.
After the initial Medical Review, a determination will be made by your FI regarding your percentage of denials or "error rate". This determines whether your CORF will be placed into additional Medical Reviews and may require that a corrective plan be submitted by your CORF. Upon acceptance by the FI, a corrective plan must actually be implemented, and another Medical Review will be initiated in order to audit your medical records for compliance.
